
Request a Transcript
Download the transcript request form. In order to fill a transcript request, we need the following information from you:
•Full name or names you attended under. If you are not sure which name your record is stored under, be sure to list all the possibilities.
•Student ID number.
•Approximate dates of attendance.
•Date of birth.
•Address to send transcript. Please be as specific as possible. The more specific the address, the greater the chance that the transcript gets to the correct person.
•Your contact address and telephone number. If we have a problem reading your request or finding your transcript, the contact information will enable us to remedy the problem quickly.
•Your signature. Federal law requires your signature for the release of your record.
We accept transcript requests in the following ways:
•IN PERSON. The Registrar’s Office is located in Alexandria Hall and is open from 8:00 a.m. to 4:30 p.m. Monday through Friday. Forms are available in our office for providing the above information.
•BY MAIL. You may send your written request containing the above information to:

There is no charge for requesting 5 or fewer copies of your transcript at one time. If you need more than 5 copies, please contact our office for payment information.
